How do I find out if my google, Facebook, and phone have been hacked?

I continue to have inappropriate subtle activity on my accounts and phone that I am not performing. My boyfriend is starting to think I'm being disloyal. I think my ex-husband is behind all this unexplainable activity on my accounts. I have had to change accounts several times due to this continuing to happen. How can I find out what IP address is linked to certain activities?
