No mobile data, phone says LTE: ESM-65540; How do I fix that?

My HTC desire 816 with virgin mobile won't connect to 4g or 3g. Mobile data hasn't worked for a week now, after my trip to mountains. I've tried soft resets and restarting but LTE eventually disconnects again no matter what. SIM card is in. Virgin Mobile customer support has no idea what's wrong; I'm in a good service area. In January I had this issue and soft reset worked after several tries.

I lose my ip address in 4G settings when the signal dies, can't manually enter one. I can't call or text, or use GPS without restarting my phone but the signal always dies anyway after 30 minutes or so.
