Unsupported SD on my s7?

So I was using my Galaxy S7 and all of a sudden it freezes on me. I was only listening to music. so I reset my phone and once it turned on it kept telling me my sd card is unsupported. I've tried reseting it and I even looked at it on a pc but on both my phone and pc it tells me to format ithat to use. I tried formatting it butyet phone and pc kee telling me that it can't be formated l. what do I do?
