Stuck in call mode.

I have been looking into this for a few hours and found nothing that helps! When I try to adjust my phone volume it automatically adjust call volume. I cannot get it to default to regular media volume. I factory reset my phone and it fixed it for a few days but then went straight back to the button only the fixing the phone call volume. I cannot receive notifications, my phone only vibrates and make a beeping sound. Sometimes when I plug my aux cord into my phone the music still plays out of the speaker. When I restart my phone it fixes it but only for about 30 min and then it goes right back. Has anyone found a solution to this problem? I hear it might be a software problem. I just got my phone replaced and all of a sudden this started happening, I never had this issue with my old phone. I have an LG V10.
